FORM CODE (DATA EMAILED) The easiest way to create a form is with Mail Dog's wizard. Depending on the type of form you wish to create, you will need to select the appropriate tags from the list below and then insert them in the text box of your template, and in the order that you wish them to appear. Once your form is created and saved, use the Landing Page URL utility to grab the form's URL so that you can link to it from your email message. Remember to also create a confirmation page that people will see when they complete and send the form. ------------------------------------------------------------------------------------------------------------ GETTING STARTED: First, you'll need to determine what information you'd like to collect and the simplest way your respondents can submit that information to you. The goal is to keep the form as short as necessary. Next, you'll need the email address that you'd like the responses to be sent to, the subject line of that email (your respondents never see these) and the URL of the confirmation page your respondents see upon submission (often this is a thank you or confirmation page). ------------------------------------------------------------------------------------------------------------ FORM HEADER: These must be the first three tags you insert in every form you create. This will trigger Mail Dog's form manager so insert them exactly as they appear below. You will need to replace the Value for the three hidden fields. Make sure you keep everything else intact including the quotation marks!
Please note! You will need to add the desired 'To' email address you are using in this form to your Mailform List on your Lists page (click the Mailform List link at the bottom of your Lists page at any time to add/remove email addresses to the list of recipients allowed to receive form data). ------------------------------------------------------------------------------------------------------------ ALIGNMENT & FONT: This tag controls the alignment, font selection and font size for the entire form. In the example below, the form will be flush left and uses Arial font, size "2". You can insert different alignments (right, left, center) and change the font and size. Leave everything else intact (including the quotation marks), and insert immediately after the Form Header tags above.

----------------------------------------------------------------------------------------------------------- FIELDS: You can have as many fields as you wish in your form. You can also label and specify the length of the field. For each field in your form, insert one of the following tags, label it accordingly and specify the size by indicating a number of characters. The "Label" is the definition of the field and needs to be entered twice. The first time is the portion that will be visible to your survey participants and the second time is the name of the field of the respondents’ answers. This is what will be visible in your returned email, so that you will know which question they are answering. You can specify the instructions at the top of the box (ex. "Enter your feedback here"). You can also specify the size in rows and columns. For each checkbox in your form, insert the following tag where you want the checkbox to appear. You can specify the alignment and label. The NAME for each checkbox will help tell you what question they're answering, the VALUE will be the actual answer. Specify the text that you want to the right of the checkbox by modifying Checkbox Text. Leave everything else intact, including the quotation marks.

The NAME for each radio button will need to be exactly the same (as radio buttons force a respondent to choose only one answer) and this NAME will help tell you what question they're answering, the VALUE will be the actual answer. Specify the text that you want to the right of the radio button by modifying Radio Text. Leave everything else intact, including the quotation marks.
